PACE — One Program That Becomes the Doctor, the Day Center, and the Ride

What it is

PACE (Program of All-Inclusive Care for the Elderly) wraps everything into one team: your doctors, a day health center with activities and meals, physical therapy, medications, rides to and from the center and appointments, even in-home help. The goal is simple — keep you living at home, out of a nursing home.

Who can get it

Age 55+, living in a ZIP code a PACE plan serves, needing nursing-home-level care (the state certifies this), and able to live safely at home with PACE's help.

What it costs

With Medi-Cal: usually nothing. With Medicare only: a monthly premium. The PACE plan explains your exact numbers before you join. One trade-off to know: you use PACE's doctors and network once enrolled.

AltaMed Senior BuenaCare · Asian Heritage Healthcare PACE · Brandman Centers for Senior Care · K-Day PACE · MyPlace LA PACE · Pacific PACE (WelbeHealth) · Seen Health (San Gabriel Valley) · Chinatown Service Center — coverage is ZIP-by-ZIP (Glendale, Burbank, Pasadena, and most of the City of LA each have several options).

How to start

  1. Open the state's PACE page and the "Service Area Counties and Zip Codes" list — find your ZIP; it names the plans you can call.

  2. Call a plan; they check eligibility, arrange the state's level-of-care certification, and walk you through enrollment. Joining and leaving are voluntary.
