Every fact on this site comes from a public government file. We show each file’s own date, so you can tell how current it is. We don’t add ratings, reviews or opinions of our own.

What’s in this build

  • 897 licensed care providers: 635 residential care facilities for the elderly, 213 home-care organizations, 49 adult day programs.
  • 1501 LA County hospice entries: 1190 with an active CDPH license, 1160 in Medicare’s hospice file.
  • 46 free public programs.

Care providers

From the California Department of Social Services’ Community Care Licensing downloads. We list facilities whose status is LICENSED or ON PROBATION in these cities: Los Angeles, Glendale, Burbank, Pasadena, La Crescenta, Tujunga, Sunland, North Hollywood, Van Nuys, Encino, Sherman Oaks, Tarzana, Northridge, Granada Hills, Montebello, Hollywood. Pending and closed facilities are left out. The state’s download carries no date of its own, so the date we show is the day the state generated the file. Visit, citation and complaint counts are copied from the same file.

Hospices

  • State license: CDPH’s Licensed and Certified Healthcare Facility Listing, rows of type HOSPICE in Los Angeles County. “Found” means the row shows an active license.
  • Medicare: CMS’s Hospice General Information file. We link a CMS record to a CDPH record by CMS Certification Number, or, when CDPH has no number on file, by exact name and ZIP code. CMS records with no match get their own page saying no state license was found.
  • Same building: street addresses compared with suite and unit numbers removed, plus the ZIP code.
  • Exclusion list: HHS-OIG’s LEIE file. Only exact name matches (ignoring punctuation and capital letters) are shown, always as “possible match — verify”. A person’s name only matches a California entry, and not if the middle initials differ.
  • State penalties: CDPH’s archived Health Facilities State Enforcement Actions file, matched by CDPH facility ID. It stops at its archive date.

A record missing from a file is a fact about the file on its date, not a finding about a business. Found something wrong? The agencies’ own lookups are linked on every page, and here is where to report concerns.
